Having appeared before a representative of the NYSRWB and waived your right to appeal, and the Board considering the circumstances, your suspension is reduced from 30 to 15 days, 7/6/09 to 7/20/09, and your fine reduced from $500.00 to $250.00. You are hereby notified that any violation of this rule, 4120.6 (1) or 4120.4, in a 2-year probationary period from the date of this notice will result in a minimum 60-day suspension. |
